How provider matching works
When you complete your ThriveAxis assessment, we match you with an independent licensed provider — an MD, DO, NP, or PA — who is actively licensed in your state and credentialed in the service you are seeking (GLP-1, TRT, BHRT, or peptide therapy). DEA registration is verified for any provider authorized to prescribe Schedule III controlled substances such as testosterone cypionate.
Before your first appointment, your treating provider's full name, credential, NPI, license state(s), license number, and DEA status (when applicable) are displayed in your patient portal. You can independently verify their license through the relevant state medical board and their NPI through the federal NPPES registry — using the lookup links below.
We display the treating provider's credentials inside your portal rather than on this public page because state advertising rules and patient-privacy norms differ on what may be published about clinicians treating individual patients. If you would like to confirm provider credentials before completing your assessment, email support@thriveaxis.org and a member of our compliance team will respond within one business day.

Active license verification
Primary-source check with the state medical board for every state the provider is licensed in. Re-verified annually.
NPI confirmation
National Provider Identifier confirmed against the federal NPPES registry. Specialty taxonomy reviewed for fit.
DEA & controlled-substance authority
For TRT and any other controlled-substance prescribing, DEA registration is verified before the provider is enabled for those services.
Disciplinary history review
Public board orders and OIG exclusion lists checked at credentialing and re-checked at re-credentialing.

ThriveAxis is currently available in 49 U.S. states. North Dakota is excluded due to current state telehealth licensure rules for one or more services on our roster.
State availability differs by service. GLP-1 therapy, TRT, BHRT, and peptide therapy each have their own state matrix based on (1) provider licensure, (2) controlled-substance authority where applicable, and (3) state-specific telehealth and pharmacy rules.
